The D-HOMES project focuses on helping people with experiences of homelessness who live with type 2 diabetes. People with experiences of homelessness are at a 3 to 6 times higher risk of dying from diabetes related issues than those with stable housing. While they face similar rates of diabetes, they often struggle more with managing their condition due to challenges like accessing medications and dealing with stress.
So far, we have worked together to design a tailored diabetes wellness coaching program called D-HOMES. Coaches provide personalized support for both English and Spanish speakers using a psychology approach called behavioral activation. These sessions aim to improve medication adherence and mental well-being, while also connecting participants to vital resources for housing, food, and health care.
Our team plans to measure how effective D-HOMES is compared to brief education and usual care at improving blood sugar control and mental well-being in the years to come.
