CV-HOMES addresses the disproportionate rates of cardiovascular (CV) disease among people experiencing homelessness, particularly within Black, Indigenous, and other communities of color in Minnesota. We focus on tailoring a training program for people at high risk for CVD to offer support for multiple health behaviors that impact CV health: not smoking, taking prescription medications, sleeping well, eating well, and staying active.
We first gathered stories (qualitative interviews), analyzed what we heard and adapted an existing coaching program (see more here).
Second, we offered our new training program to people in supportive housing with recent homelessness (open trial) and began to develop impact measures on CV risk and psychological well-being.
